Cognitive First/CogRead Literacy Campaign School Application

A school principal, school district administrator or authorized representative of either can complete the form below as an application for sponsored access. The request is for (1) Sound Analysis in order to better prepare K-1 children for phonics and reading instruction and/or (2) to be considered for Cognitive Skill Screening for every 2nd grade and above student. View the introductory video, the Gibson Test. Individual schools that complete the cognitive skill testing of all of their 2nd grade and above students can qualify for a Turning Point brain training intervention package for ten (10) free and reduced lunch qualified students.

Completing and submitting this form does not represent a commitment on the part of the school or on the part of Cognitive First or the CogRead Literacy Campaign. Your application will be considered for approval based upon the then current availability of sponsorships and your school’s informed desire, commitment, and capacity to implement the requested programs. Once your school has been approved your admin database group will be set up and the agreed upon sponsorship offer can begin.
